IF THIS IS YOUR FIRST SICK NOTE, HAVE YOU:
1. Been in hospital?
If you have been in hospital and need to stay off work- the hospital is responsible for issuing your sick note, covering the full period until the date by which it is anticipated that you will be able to return to work.Please contact the hospital if they did not provide you with a sick note.
2. Been unwell for less than 7 days?
If you have been unwell for less than 7 days you do not need to see a GP. You can complete a Self Certification form yourself. However, some employers insist on a Doctor's note regardless. A Doctor's note under these circumstances involves a charge of £20. Many employers have their own self-certification forms.If your employer doesn't have it's own form you can download one one by clicking on the link below. Please print, complete and hand to your employer.

3. Been unwell for more than 7 days?
Please download the form below, complete it and drop it into the surgery. The completed form will be ready for collection within five working days. We will send you a text message when the form is ready to collect.
OR IF YOU HAVE ALREADY HAD A SICK NOTE FOR THIS ILLNESS:
Your Doctor may not need to see you to issue an additional sick note. Please download the form below, complete it and drop it into the surgery. We will either send you a text message when the form is ready to collect or contact you to make an appointment to see a GP.
